A 27-year-old female presents with a 7-year history of decreased vision in both eyes. She has no other symptoms or complaints. Preceding ocular, medical, and social histories are all unremarkable. Best corrected visual acuity is 20/40 right eye and 20/32 left eye. Slit lamp examination reveals the findings seen in these photos of the right and left eyes respectively. Posterior segment examination was normal. What could be happening?

En el caso de esta paciente se observan en las fotos expuestas opacidades corneales centrales, bilaterales que recuerdan las distrofias corneales.
Dada el tiempo de los síntomas y manifestaciones clínicas debemos pensar y descartar la distrofia endotelio de Fuchs.

…but another indication for corneal biopsy would be diagnosis of corneal dystrophies & metabolic storage disease with corneal manifestations.
I humbly feel this will ACCURATELY diagnose these corneal lesions.

I would certainly agree that corneal biopsy may be definitively helpful in some cases, particularly in cases of refractory infectious keratitis and metabolic diseases with corneal manifestations. However, in this particular case, with an otherwise normal patient, relatively good vision, and no suspicion of an infectious process, corneal biopsy is probably a bit aggressive given the amount of tissue taken and accompanying risk of perforation.
